Heal With Zainab offers health coaching, yoga and wellness services in Lahore, Dubai and online. This policy describes information collected and used through healwithzainab.com and its enquiry and consultation forms.
What information we collect
The Contact and Book a Consultation forms collect details such as your name, email, phone number, service interest and message. The Free Health Coaching Consultation form also asks about health concerns, medical and medication history, age, date of birth, family background, sleep, diet, lifestyle and other information relevant to coaching. Questions depend on the selected adult, senior, teen or child profile.
Why we use your information
We use the information to review and respond to enquiries, prepare for consultations, discuss suitable services, arrange appointments and maintain related records and website security. Completing a questionnaire does not automatically confirm a booking. Health coaching does not provide medical diagnosis or emergency treatment.
Where consultation information goes
Free consultation questionnaires are stored as private records in the site’s WordPress administration area. A notification containing the complete submitted health questionnaire is sent to the site’s configured administrator email address. The applicant also receives a confirmation email, without the complete questionnaire. Contact and booking forms email the submitted message and contact details to the site administrator. Hosting and email providers may process these records and messages to deliver these services.
Only authorised personnel should access administrative records and consultation email accounts. Ordinary email can present privacy and security risks. Do not submit detailed medical histories through the general Contact or Book a Consultation forms, and do not use any form for emergencies.
Children and teenagers
The intake form includes teen and child profiles and requests a parent or guardian name and email, together with explicit guardian consent. Parents or authorised guardians should supervise submissions. If you think a minor’s information was submitted without proper permission, please contact us promptly.
